LPN (Licensed Practical Nurse)

· Receive and give report at the beginning and end of shifts

· Round with DON,MDS, Wound Nurse every morning

· On call as scheduled

· Administer medications as prescribed in a timely manner

· Make sure all medications are given and signed out correctly prior to the end of your shift

· Check the cart daily for any expired or discontinued medications and remove from cart

· Check the med room and stock meds for any expired or discontinued and send back to pharmacy appropriately

· Print No BM report daily and provide medication as needed

· Check the refrigerator temperatures daily and record in the med room

· Make sure proper documentation is completed for removal of ER box medications

· Order medications daily as needed to ensure cart is stocked

· Complete incident reports for any incident occurring on your shift and document

· Document any refusal or behaviors that occur during your shift in the resident’s chart

· Assist Charge nurse with acute board charting if needed, particularly on provider rounding days.

· Help ensure that all residents are out of bed, clean, shaved, items stored correctly, etc.

· Ensure any diabetic resident has their own personal glucometer and that boxes are stocked, report to DON if patient does not have own glucometer.

· Make sure vitals and weights are entered on any admissions that occur during your shift

· Any other task that you may be asked to complete by the administrator or DON or Charge Nurse

· CNA assignments

· Monitor O2 sats on all residents on O2 each shift.

· Address any new concerns with residents, skin concerns, obtain orders, and then report to charge nurse.

You are held to the expectation to perform within your scope as a nurse and to complete duties specific to your profession each shift

Pay: $30.00 - $35.00 per hour
