Responsibilities

• Provide direct patient care to our residents, including administering medications, performing vital signs, and providing wound care
• Collaborate with the interdisciplinary team to develop and implement individualized care plans that promote the interests and well-being of our residents
• Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes
• Participate in quality improvement initiatives to ensure that our care is evidence-based and resident-centered
• Provide emotional support and comfort to our residents and their families, and engage in activities that promote socialization and community building
• Demonstrate a commitment to our organization's values, including delivering excellent customer service, promoting the success of others, leading by example, and committing to quality outcomes

Qualifications

• Current New York State Licensed Practical Nurse (LPN) License
