Responsibilities:

LPN is responsible for traveling to a patient’s home to administer their services and helping patients maintain their independence. Their duties include administering at-home IVs, changing dressings, cleaning wounds, medication management and updating Doctors about their patient’s health.

• Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
• Education to patient and family in teaching sings and symptoms of illness.
• prepare clinical notes for the clinical record and perform clinical assessments independently.
• Maintain a safe therapeutic environment for clients.
• Apply concepts of infection control and universal precautions in coordination with performing patient care activites to prevent contamination and transmission of disease.
• Maintain accurate and up-to-date patient records.
  -Review the patient plan of care, progress notes, phone orders and other clinical record documentation on an ongoing basis.

Qualifications:

• Valid LPN license in Illinois.
• Minimum [1] year of experience as an LPN in Home Health Care setting.
• Current CPR certification.
• Experience working with patients in Home Health and providing care is preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Reliable transportation required with current Illinois Drivers License and liability insurance.
