LPN Med Care Manager

Role Overview The LPN Medication Nurse provides licensed nursing support in a senior living setting, ensuring resident safety through safe medication administration, health monitoring, and accurate clinical documentation.

Key Responsibilities

• Administer medications and treatments according to physician orders
• Follow the Six Rights of Medication Administration to prevent errors
• Document accurately in MAR/TAR in real-time
• Monitor symptoms and administer medications for symptom relief
• Report refusals or adverse reactions promptly
• Conduct Service & Health Updates (SHUs) and transcribe physician orders
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts, complete narcotic counts, and ensure compliance standards

Qualifications & Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation abilities
• Experience in senior care or medication administration preferred
