We’re looking for a caring, dependable, and skilled LPN to support a client in their home in Ostrander, Ohio.

This role is about more than providing clinical care—it’s about getting to know your patient, building trust, and helping them feel safe, comfortable, and supported each day. If you’re a nurse who takes pride in treating every patient with dignity, patience, and compassion, we’d love to hear from you.

What You’ll Do

• Provide individualized, patient-centered nursing care in the home
• Provide safe and attentive G-tube care
• Administer medications and treatments as ordered
• Monitor the client's condition and communicate changes to the care team
• Follow the individualized plan of care
• Maintain accurate clinical documentation
• Create a calm, respectful, and supportive environment
• Build a positive relationship with the client and family

Schedule

Weekends | 8:00 AM–8:00 PM

A consistent weekend schedule that allows you to provide meaningful, uninterrupted care while building a strong connection with your patient.

What We’re Looking For

• Active Ohio LPN license required
• G-tube experience required
• Compassionate and patient-centered approach
• Strong clinical skills and attention to detail
• Dependable and professional work ethic
• Comfortable working independently in a home setting
• Reliable transportation
• Ability to pass required background and drug screening
