We are personal home care agency located in Duluth, GA. We provide in home care throughout the North Atlanta Area. We specialize Medical and Non Medical services for Stroke Recovery, Traumatic Injury Recovery, Elder Care and Veteran Care.

Description

• Maintaining client’s progress notes and clinical notes if client is receiving skilled nursing care.
• Ensuring care continuation in case of client transfer.
• Coordinating care services between iProvide Home Care Solutions, LLC and other healthcare providers when necessary.
• Preserving the privacy/confidentially of client information and agency matters.
• Maintaining accurate time records and submitting required records on time
• Responding to emergencies and participating in weekend and holiday shifts/schedules.
• Taking part in any special projects, including surveys and studies.
• Supervising direct care worker/caregiver dress code and attendance.
• Reporting all suspected or witnessed cases of abuse and serious incidents to the relevant bodies as directed by rules and regulations.
• Taking part in policy formation and revision.
• Assisting the Administrator to make decisions related to care provision and services.
• Complying with and ensuring compliance with agency policies and procedures.
• Protecting iProvide Home Care Solutions, LLC’s resources, supplies, equipment, and other property.

Qualifications and Education Requirements

• Must have a bachelor’s or master’s degree in nursing from a recognized institution.
• Must have a valid LPN state licensure.
• Must have First Aid and CPR certifications.
• Must have 1-3 years of nursing experience, preferably in a home care setting.
• Great organizational and supervisory skills will be an added advantage.
• Must be able to think and act quickly in case of an emergency.
• Must be kind, compassionate, and non-judgmental.
• Must be computer literate and be able to communicate well in both verbal and written English.
• Must be a team player and willing to respect and work with clients and workmates.

Must be willing and able to work under stressful conditions with or without direct supervision.

• Must have a full understanding of the rules and regulations pertaining to the industry.
• Must be willing and ready to comply with State and Federal laws, and agency policies as well.
• Must be willing and ready to avail all records in a timely way.
• Must be ready to demonstrate a full understanding of Universal infection control and OSHA procedures.
• Must have the physical ability to stand for long hours and to lift or support clients who have mobility issues.
• Must possess excellent visual and auditory senses.
• Must pass a background screen.
• Must be willing and ready to travel as required, and be in possession of a valid driver’s license and automobile insurance.
• Must provide evidence of adequate health status to perform duties.

This position has potential for cross-training to other programs within the agency. Cross-training to perform those job functions is not included in the home care job description. This portion of cross-training will be utilized on an as needed basis.

Physical Demands

Our LPN many spend long hours on his/her feet, and, therefore, requires stamina. He/she is also likely to help lift patients and support those who need physical assistance to walk, rise from the bed or toilet seat, etc. Based on this and other special circumstances, the Physical Demand relating to this job position include:

1. Standing and sitting for more than 20 minutes.

2. Pushing, pulling, lifting, crawling, crouching, kneeling, climbing, and stooping.

3. Touching, feeling, holding, and handling.

4. Talking and listening.

5. Walking and running.

6. Tasting and smelling.

7. Close vision, peripheral/indirect vision, distant vision, and depth perception.

Salary

$28 - $35 per hour
