Lakeview Personal Care LPN Opportunities:

• LPN, Shift Supervisor - Full Time Day 6:45am-3:15pm 64 hours/2 weeks including every other weekend & holiday ($31.56 - $34.85/hr)
• LPN, Shift Supervisor - Part Time Day 6:45am-3:15pm 32 hours/2 weeks including every other weekend & holiday ($31.56 - $34.85/hr)
• LPN, Shift Supervisor - Part Time Night 10:45pm-7:15am 32 hours/2 weeks including every other weekend & holiday ($33.56 - $35.60/hr)

LPN Team Leader Responsibilities include:

• Monitors and provides for the medical needs of the resident through emergency response, physician contact, accurate and complete clinical assessments, requests for physician services, pharmacist consultations, and other measures as indicated.
• Performs treatments as prescribed. Directs Med Tech’s assignments when delegating.
• Responsible for supervision of Med Techs and Resident Assistants. Coordinates resident care and manages staff duties and assignments, provides feedback and counseling to team members, etc.
• Assists residents in maintaining health through education (nutrition, hygiene, exercise, medications) and advocacy for residents with other health care providers.

Requirements

LPN Team Leader Qualifications:

• PA Licensed Practical Nurse license required.
• Experience working in geriatrics and/or those with dementia is helpful.
