As a Manager (Partnership & Community Development), you will be responsible for the following duties:

• Support the development and execution of strategies aimed at empowering the community to take charge of their own health and well-being.
• Develop and implement community health plans, focusing on preventive care and the integration of health and social services.
• Monitor and evaluate the progress of the community health plan implementation across various focus areas.
• Foster engagement and build partnerships with key stakeholders such as grassroots organisations, social service agencies, and healthcare providers to enhance population health and ensure coordinated care across settings.
• Provide secretariat support for meetings, including scheduling, documentation, and follow-up.

Requirements:

• Possess a Bachelor’s degree in Business or a related field
