Position Summary

We are seeking a Marketer to join our team and play a dual role in referral source management and patient intake. This position relies on relationship-building skills to ensure we maintain close relationships with referral sources and build strong relationships with new referral sources.

Key Responsibilities

• Manage incoming referrals from hospitals, skilled nursing facilities (SNFs), case managers, and other partners.
• Generate new referral sources.
• Perform patient intakes and execute service agreement contracts as needed.
• Coordinate care initiation promptly to reduce delays and improve patient experience.
• Maintain accurate referral and intake trackers.
• Build and maintain strong relationships with referral sources through regular communication and check-ins.
• Generate new referrals from established referral sources.
• Identify opportunities for growth and assist with marketing initiatives as needed.
• Serve as a liaison between referral sources, patients, and internal care teams.

Qualifications

• Strong organizational and time-management skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage multiple priorities.
• Previous experience in home health, personal care, or case management preferred.
• Marketing or referral development experience is a plus (but not required).

Compensation

• Hourly pay, with mileage reimbursement for intakes and referral check-ins (details discussed during interview).
