MDS Assistant - LPN

MDS ASSISTANT JOB DUTIES

(duties include, but are not limited to)

· Update and complete resident’s discharges and re-entries in computer

· Attend all morning stand-up meetings and gather information on resident’s status

· Attend admission meetings and gather data

· Attend all staff in-service meetings

· Gather information from RN. LPN, DCW and Residents for assessments

· Complete user defined assessments: Pain, Aims, and Bowel and Bladder Assessment

· Complete an MDS Note with each assessment done

· Attend weekly care plan meetings and complete a Care Conference note in computer

· Identify problems/solutions with IDT members

· Follow monthly MDS schedule and complete assessment in the computer

· Update and modify the care plans according to the MDS schedule and prn

· Identify problems and solution with IDT members

· Update orders from medical records in the care plan section weekly and prn

· Attend weekly wound/skin/nutrition/fall meetings

· Dining room duties as scheduled

· On call as scheduled

· Work a medication cart as needed

· Complete information daily on assessments open and take to stand-up meeting

· Enter CNA Care Plan Task in computer with each resident admitted and keep updated

Pay: $52,175.00 per year
