MDS/ Care Plan Coordinator

Washington Senior Living is looking for a knowledgeable, compassionate, and experienced MDS Coordinator to join our team!

We’re looking for someone who knows MDS, understands the importance of accurate care planning, and enjoys working alongside a great interdisciplinary team. What We’re Looking For

• Previous MDS experience in a skilled nursing or long-term care setting required
• Active Illinois RN or LPN license
• Knowledge of MDS assessments, care planning, PDPM, Medicare, and Medicaid
• Strong organizational skills and attention to detail
• Someone who is dependable, collaborative, and truly cares about our residents

What You’ll Do

• Coordinate and complete MDS assessments accurately and on time
• Develop and maintain individualized resident care plans
• Work closely with nursing, therapy, social services, and the interdisciplinary team
• Review documentation to help ensure accuracy, compliance, and appropriate reimbursement
• Participate in care plan meetings and resident reviews
• Help support quality initiatives, audits, and survey readiness
