Job Summary:

Vivo Healthcare Normandy is seeking a highly skilled and dedicated MDS (Minimum Data Set) Coordinator to join our team in Jacksonville, Florida. As a key member of our healthcare organization, the MDS Coordinator will be responsible for ensuring accurate and timely data collection, reporting, and submission to various healthcare organizations and regulatory bodies. Responsibilities:

• Collect, analyze, and submit accurate and complete MDS data for residents in accordance with federal and state regulations
• Collaborate with interdisciplinary teams to ensure comprehensive care planning and accurate data collection
• Identify and resolve data discrepancies and discrepancies in a timely manner
• Develop and maintain strong relationships with healthcare providers, caregivers, and family members to obtain accurate information
• Participate in quality improvement initiatives to improve MDS data accuracy and completeness
• Stay up-to-date with changing regulations and guidelines related to MDS data collection and submission

Requirements:

• High School Diploma or Equivalent required; Associate's or Bachelor's Degree in Healthcare, Nursing, or related field preferred
• 1-2 years of experience in MDS data collection or a related field
• Strong analytical and problem-solving skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced environment with frequent deadlines
• Proficiency in electronic health records and data management systems
