MDS Coordinator

Catholic Health Services seeks an MDS Coordinator for St. Anne's Nursing Center & Residence. In this on-site role, you will lead the MDS/RAI process, complete timely and accurate resident assessments, and develop person-centered care plans that reflect Catholic Health Services' mission of compassionate, holistic care. You will collaborate closely with nursing, therapy, and interdisciplinary teams, ensure regulatory compliance and reimbursement accuracy, monitor quality indicators, educate staff on documentation standards, and support continuous improvement in resident outcomes and survey readiness.
