We are currently seeking an M.D.S. Coordinator to join our nursing team and play an important role in coordinating resident assessments, supporting quality of care, and ensuring accurate and timely documentation.

What You'll Do

· Coordinate and schedule required MDS and RAI assessments in accordance with state and federal requirements

· Coordinate initial, quarterly, annual, and other required MDS assessments

· Ensure resident assessments, care plans, and supporting documentation are accurate and up to date

· Coordinate, encode, and electronically submit required MDS, Medicare, Medicaid, OBRA, and other mandated documentation

· Monitor nursing documentation to ensure it is clear, accurate, timely, and reflective of the care provided

· Coordinate reviews related to Medicare and other coverage requirements

· Work closely with the Director of Nursing, nursing staff, residents, families, and other members of the care team

· Assist with staff education related to ADL documentation and MDS requirements

· Maintain an accurate and up-to-date Roster Matrix

· Help ensure resident needs are accurately reflected through assessment, documentation, and care planning

· Communicate effectively with residents, families, coworkers, leadership, and other members of the care team

· Maintain confidentiality and follow all applicable privacy, regulatory, and facility policies

· Assist the Director of Nursing with other duties as assigned

Qualifications

· Active and unrestricted Iowa RN or LPN license required

· Previous MDS experience preferred, but not required

· Experience in restorative, geriatric, or psychiatric nursing preferred

· Previous supervisory experience preferred

· Strong organizational and time-management skills

· Excellent written and verbal communication skills

· Strong attention to detail and ability to manage multiple deadlines and priorities

· Computer proficiency and ability to learn new software and technology

· Ability to work independently while also collaborating effectively with an interdisciplinary team

· Professional, dependable, and resident-focused approach to care
