MDS Coordinator

Job Summary
We are seeking an experienced MDS Coordinator to join our team. The ideal candidate will have a nursing background and familiarity with pcc, and other healthcare systems. This role involves ensuring accurate and timely completion of Minimum Data Set (MDS) assessments in compliance with regulatory requirements.

Responsibilities

• Coordinate and complete MDS assessments for residents in various settings such as inpatient, home care, and Level II Trauma Center
• Utilize clinical documentation improvement practices to enhance accuracy of patient records
• Collaborate with interdisciplinary teams to ensure MDS assessments reflect the patient's needs and care provided
• Oversee discharge planning processes to facilitate smooth transitions of care
• Maintain knowledge of industry standards and regulations related to MDS assessments

Experience

• Previous experience as a nurse or in a similar healthcare role
• Registered Nurse
• Familiarity with MDS assessment processes in different healthcare settings such as OR, inpatient, and home care
• Strong understanding of clinical documentation improvement practices
• Ability to work effectively in a fast-paced environment while maintaining attention to detail

Job Type: Full-time

Pay: $60.00 - $65.00 per hour
