Job Summary
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. In this vital role, you will be responsible for managing the Minimum Data Set (MDS) assessments, ensuring accurate and timely completion of resident evaluations in accordance with federal and state regulations. Your expertise will support quality patient care, compliance with Medicare and Medicaid requirements, and facilitate effective clinical documentation improvement. This position offers an exciting opportunity to contribute to long-term care facilities, skilled nursing centers, or hospital settings by coordinating comprehensive assessments that influence care planning and reimbursement processes.

Responsibilities

  • Conduct comprehensive MDS assessments for residents, ensuring adherence to clinical documentation standards and regulatory guidelines such as CMS (Centers for Medicare and Medicaid Services) requirements.
  • Collaborate with interdisciplinary teams—including nursing, therapy, social services, and medical staff—to gather accurate clinical information necessary for assessment completion.
  • Review medical records, physician notes, and other documentation to verify accuracy and completeness of resident data.
  • Ensure timely submission of MDS assessments within mandated deadlines to support appropriate reimbursement and quality reporting.
  • Maintain detailed documentation review logs, audit trails, and compliance records related to MDS processes.
  • Stay current with updates in healthcare regulations, long-term care policies, ICD coding standards (ICD-9, ICD-10), CPT coding, DRG classifications, and HEDIS measures to ensure ongoing compliance.
  • Assist in training staff on documentation standards, coding practices, and utilization management protocols to enhance clinical documentation improvement initiatives.
  • Support discharge planning, case management activities, and coordination of care transitions by providing accurate assessment data.

Experience

  • Proven experience in medical documentation review within long-term care or hospital environments with a focus on MDS coordination or clinical assessment roles.
  • Strong knowledge of healthcare regulations including Medicare regulations, HIPAA privacy rules, state healthcare policies, and long-term care regulations.
  • Familiarity with EMR/EHR systems such as Cerner, Epic (Epic), Athenahealth, eClinicalWorks or similar platforms used for electronic health records management.
  • Demonstrated proficiency in medical coding (ICD-9/ICD-10), CPT coding, DRG assignment, and utilization review processes.
  • Experience working in acute care settings such as ICU or Level I Trauma Centers is highly desirable.
  • Knowledge of physiology, anatomy, medical terminology, and clinical documentation standards essential for accurate assessments.
  • Prior nursing home experience or familiarity with discharge planning, hospice care, outpatient services or pediatric settings is advantageous.
  • Ability to interpret complex health information while maintaining compliance with HEDIS standards and NCQA guidelines.
