Responsibilities:

• Conduct Resident Assessments, develop and implement individualized care plans, and collaborate with the interdisciplinary team to ensure seamless care delivery
• Identify and report changes in patient condition, concerns, or needs to the healthcare team
• Develop and maintain accurate, up-to-date records of patient assessments, care plans, and progress
• Participate in quality improvement initiatives, quality of care improvement projects, and regulatory compliance
• Provide guidance and support to other nursing staff, students, and volunteers
• Foster a culture of open communication, respect, and compassion within the team

Requirements:

• MDS certification or willingness to obtain within a specified timeframe
• Current nursing licensure in the state of Texas
• Minimum 0 years of experience in a long-term care setting (new graduates welcome to apply)
• Strong communication, interpersonal, and problem-solving skills
• Ability to work effectively in a fast-paced environment with multiple priorities
