Complete Healthcare Connect (CHC) is seeking an experienced Medical Practitioner to join our team delivering the Maintenance in the Home Program across Brisbane Metro North.

A Flexible Medical Role

This opportunity is particularly suited to experienced Medical Practitioners seeking greater flexibility, including doctors balancing family or caring responsibilities, an existing practice, or those who prefer an alternative to traditional GP hours.

Clients are generally supported for up to 12 weeks, with the Medical Practitioner providing medical oversight throughout their episode of care. This typically involves weekly clinical reviews/check-ins, with telehealth available where clinically appropriate.

This is not a high-volume GP clinic role and does not require continuous daily attendance. CHC's nursing and allied health teams provide ongoing care, escalating medical concerns when required.

Key Responsibilities

• Assess referred clients for medical suitability.
• Provide medical oversight throughout the client's approximately 12-week episode of care.
• Conduct initial reviews and typically weekly clinical check-ins.
• Provide telehealth or face-to-face reviews according to clinical need.
• Review diagnoses, medications, comorbidities and clinical risks.
• Provide medical guidance to CHC's nursing and allied health teams.
• Respond to escalated clinical concerns and support appropriate hospital or medical escalation.
• Collaborate with the multidisciplinary team.
• Maintain appropriate clinical documentation and contribute to discharge and transition planning.

What We're Looking For

• Current unrestricted AHPRA registration as a Medical Practitioner.

• Ability to meet relevant credentialing requirements.

• Experience in general practice, geriatrics, hospital medicine, rehabilitation, community or transitional care is highly regarded.

• Strong clinical judgement and confidence managing clients with chronic or complex health conditions.

• Excellent communication and multidisciplinary collaboration skills.

• Appropriate professional indemnity insurance and compliance requirements.
