Medical Coder

Key Responsibilities

• Review patient medical records, clinical documentation, and physician notes.
• Assign appropriate diagnosis and procedure codes.
• Ensure accurate and complete coding based on available documentation.
• Follow applicable coding guidelines, payer requirements, and compliance standards.
• Identify documentation gaps and coordinate for clarification where required.
• Review claims and coding-related discrepancies.
• Maintain confidentiality and security of patient information.
• Meet defined productivity, accuracy, and quality targets.
• Participate in quality audits and corrective-action activities.
• Stay updated with changes in coding guidelines and healthcare regulations.

Required Skills

• Medical coding and healthcare terminology.
• ICD, CPT, and HCPCS coding knowledge as applicable.
• Anatomy and physiology knowledge.
• Medical record/documentation review.
• Strong attention to detail and analytical skills.
• Knowledge of healthcare claims and billing processes.
• Good computer and documentation skills.
