What they need

Chartiers Center is a private, non-profit behavioral health care organization serving Southwestern Pennsylvania.  Our mission is to provide compassionate support to those in need who face behavioral health, co-occurring disorders or intellectual/development disability (IDD) challenges as they transform our lives.

As a Mobile Case Manager with Chartiers Center, you will have the opportunity to make a meaningful impact in the lives of adults living with serious mental illness and co-occurring conditions, including substance use disorders, and physical health challenges. In this dynamic, community-based role, you will build supportive relationships, connect individuals with essential resources, and serve as an advocate for their well-being and success.

If you are passionate about helping others, enjoy working independently while staying connected to a collaborative team, and want to be part of an organization dedicated to recovery and hope, this is the position for you. Job Duties Include:

  • Meet with individuals in their homes and throughout the community to provide support, guidance, and service coordination.

  • Conduct comprehensive assessments to identify strengths, needs, preferences, and recovery goals.

  • Develop, implement, and monitor individualized service plans that promote stability, growth, and independence.

  • Collaborate with healthcare providers, therapists, family members, and community agencies to ensure coordinated and effective care.

  • Maintain regular contact with individuals to monitor progress, address challenges, and identify emerging needs.

  • Serve as a strong advocate, helping individuals navigate systems and access the services they need to succeed.

  • Complete accurate and timely documentation in accordance with agency and regulatory requirements.

  • Participate in a shared on-call rotation, typically no more than once per month.

Successful Candidates Demonstrate:

  • A compassionate, recovery-oriented, and person-centered approach to supporting others.

  • Excellent communication and interpersonal skills, with the ability to build trust and rapport.

  • Strong organizational skills and the ability to effectively manage priorities in a fast-paced environment.

  • Confidence working independently while providing services in community-based settings.

  • The ability to develop positive relationships and collaborate with individuals, families, providers, and community partners.

  • Initiative, self-motivation, sound judgment, and strong problem-solving abilities.

  • A genuine passion for helping individuals achieve recovery, independence, and an improved quality of life.

This is an excellent opportunity for a caring and motivated professional who wants to make a meaningful difference every day while being part of a supportive team dedicated to empowering individuals and strengthening our community.
