Mobile Family Nurse Practitioner

Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our new and rapidly growing mobile team. The ideal candidate will provide in home comprehensive care to patients of all ages, focusing on health promotion, disease prevention, and management of acute and chronic illnesses. This role requires a strong foundation in medical knowledge and the ability to work collaboratively within a multidisciplinary team to ensure optimal patient outcomes.

• Looking for two providers who live: in Phoenix, Scottsdale and East Valley
• Looking for one provider in Bullhead City, Kingman, Fort Mohave
• This is a 1099 contract position
• Must be available 2-3 days per week or see 20-40 patient visits per week/80-160 patient visits per month
• Care delivery will take place in patient homes, assisted living facilities, and group homes.

Responsibilities

• Conduct thorough patient assessments, including medical histories and physical examinations.
• Diagnose and manage a variety of health conditions, utilizing evidence-based practices.
• Develop and implement individualized treatment plans based on patient needs.
• Prescribe medications and therapies as necessary, with a focus on psychopharmacology when applicable.
• Provide education to patients and families regarding health maintenance, disease prevention, and treatment options.
• Collaborate with other healthcare professionals in hospital medicine and critical care settings to coordinate patient care.
• Maintain accurate medical records and documentation in compliance with healthcare regulations.
• Stay current with advancements in medical terminology, physiology knowledge, and best practices in family medicine.

Requirements

• Master’s degree in Nursing from an accredited program.
• Current certification as a Family Nurse Practitioner (FNP) by a recognized certifying body.
• Proficiency in medical terminology related to family practice and acute care settings.
• Excellent communication skills, both verbal and written, with the ability to educate patients effectively.
• Ability to work collaboratively within a healthcare team while demonstrating strong leadership qualities.
• Commitment to providing high-quality patient-centered care in a fast-paced environment.
• Must have reliable transportation
• Will need to acquire their own equipment to perform job duties, such as blood pressure cuff, pulse ox, stethoscope, and thermometer
• Be available 2-3 days per week
