NDIS Support Coordinator

About the role

Alphacare Services is seeking an experienced and dedicated NDIS Support Coordinator to join our team in Melbourne. This full-time role will be responsible for providing high-quality support coordination services to clients with disability, helping them to access and utilise their NDIS funding effectively.

Salary Range: $83,000 – $90,000 per year

What you'll be doing

• Develop and implement comprehensive support plans to meet the individual needs of NDIS participants
• Coordinate access to a range of support services, including allied health, accommodation, employment, and community engagement
• Advocate on behalf of clients to ensure they receive the appropriate level of funding and support
• Regularly monitor and review client progress, making adjustments to support plans as needed
• Maintain detailed records and provide regular reports to clients, their families, and the NDIS
• Collaborate with a multidisciplinary team of health and community professionals
• Contribute to the continuous improvement of our NDIS support services

What we're looking for

• Minimum 2 years' experience in NDIS support coordination or a related field
• Thorough understanding of the NDIS framework, operational guidelines and eligibility criteria
• Excellent communication and interpersonal skills, with the ability to build rapport with a diverse range of clients
• Strong problem-solving and decision-making skills, with a focus on achieving positive outcomes for clients
• Demonstrated ability to work independently and as part of a team
• Diploma of Community Services or relevant tertiary qualification in disability services, social work, or a related field
• Current driver's licence and access to a reliable vehicle

We are a NDIS registered organisation providing support and services with high standards to improve the quality of life, develop the essential skills, build confidence and engage them in social and community activities

We provide Core Supports and Support Coordination, Short Term Accommodation (STA), and supported independent living (SIL) for eligible NDIS participants.
