What they need

Company Description

Here2Home provides NDIS Specialist Support Coordination, support coordination, social work, and plan management to participants of the National Disability Insurance Scheme (NDIS). Our services are designed to help individuals achieve their goals and improve their quality of life. Our team of dedicated professionals works diligently to ensure every participant receives tailored assistance that addresses their unique needs and circumstances.

Role Description

There is multiple sub-contracting roles for an NDIS Level 2 or Level 3 (Specialist) Support Coordinator. The Support Coordinator will be responsible for coordinating support services, facilitating communication between participants and service providers, report writing and delivering exceptional customer service. Day-to-day tasks include developing and managing participant support plans, connecting participants to services, tracking budgets, attending meetings, report writing, and case noting. If you are interested in Level 3 Specialist Support Coordination you must hold an allied health degree or relevant industry experience that is deemed acceptable by Here2Home (ie complex case management overseeing high risk etc).

  • Whilst home visits are not a regular requirement, please do not apply if you are unwilling to travel to visit participants.
  • Whilst experience as an NDIS support coordinator is not essential, it will be highly advantageous and looked upon favourably.
  • Must have or be willing to obtain an NDIS Clearance and Working With Children Check.
  • Ensure you send your CV along with a cover letter to [email protected]. Applications missing these 2 requirements will not be considered.
  • Immediate start - do not apply if you are unable to start within 2 weeks post position offer.

*Important: We are seeking someone who is genuinely committed to this role for the long term. Applicants must be willing to commit to a minimum of 12 months, as our participants rely on stable, ongoing relationships. Changes in support coordination can be highly distressing and disruptive for them.

Qualifications

  • Excellent in communication skills with people with disability and stakeholders
  • Proficiency in report writing
  • Ability to work independently and remotely
  • Willingness to conduct home visits to participants
  • Strong organisational and time management skills
  • Relevant experience in the disability or support sector
