Responsibilities:

• Provide direct patient care to a diverse range of residents, including those with complex medical needs and ongoing rehabilitation goals
• Develop and implement personalized care plans in collaboration with the interdisciplinary team, ensuring a comprehensive approach to each resident's care
• Administer medications, treatments, and therapies as ordered by physicians and prescribed by healthcare providers
• Monitor and report patient vital signs, medical conditions, and behavioral changes to the healthcare team
• Assist with the transfer and mobilization of residents, as well as provide emotional support and companionship during difficult times
• Maintain accurate and detailed records of patient care, progress, and outcomes
• Participate in care conferences and interdisciplinary team meetings to discuss patient care and progress
• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care
