NJDDDD Support Coordinator

We are seeking a compassionate, organized, and motivated Support Coordinator to join our growing team serving individuals with intellectual and developmental disabilities (I/DD) throughout New Jersey. If you have experience helping individuals and families navigate services, coordinating supports, and advocating for meaningful outcomes, we'd love to hear from you. This is an excellent opportunity for professionals with backgrounds in developmental disabilities, CSOC, social work, case management, or behavioral health who are looking to make a lasting impact.Responsibilities

• Coordinate and monitor services for individuals with intellectual and developmental disabilities.
• Conduct monthly contacts, home visits, and person-centered assessments.
• Develop and maintain individualized service plans.
• Collaborate with families, providers, and interdisciplinary teams.
• Connect individuals with community resources and available supports.
• Maintain accurate and timely documentation in accordance with NJ DDD requirements.
• Advocate for individuals to ensure they receive appropriate services and supports.

Required Qualifications

• Minimum 1 year of experience working with individuals with developmental disabilities (DD).
• Bachelor's degree in Social Work, Psychology, Human Services, Special Education, Counseling, or a related field (or equivalent experience if permitted by NJ requirements).
• Excellent communication, organizational, and time management skills. Must possess a valid driver's license, reliable vehicle, and ability to successfully complete all required background screenings.
• Ability to work independently while managing a caseload.

Preferred

• Experience working within CSOC/PerformCare.
• Background in social work, case management, behavioral health, or related human services.
• Knowledge of NJ DDD programs and community resources.

Work Location: Hybrid - requires community visits, in person meetings, and remote documentation/work from home

Pay: $25.00 - $28.00 per hour

Expected hours: No more than 35.0 per week
