What if your next nursing role could change the course of a young person’s life?
Great nursing care goes beyond the medical chart. It’s about advocating for young people, coordinating essential care, and creating safer, healthier environments where they can thrive. Key Assets Kentucky is seeking a compassionate, organized, and experienced Nurse Care Coordinator (RN) to oversee and coordinate medical care for youth in our Children in Community Care (CCC) residential program.
This is not a traditional hospital or clinic-based nursing role. The youth we serve live in Key Assets Kentucky’s residential group homes and may have complex medical, behavioral, and emotional needs that require individualized, consistent, and compassionate care.
If you’re passionate about advocating for vulnerable youth and making a meaningful difference beyond a traditional healthcare setting, this could be an excellent fit for you!
What You'll Do
- Coordinate medical care and appointments for youth living in our residential group homes
- Conduct monthly health assessments and monitor ongoing healthcare needs
- Oversee medication management, including prescriptions, medication administration records, and compliance
- Serve as a liaison between healthcare providers, youth, and the CCC treatment team
- Maintain accurate medical records, documentation, and treatment-related information
- Provide medication administration and CPR/First Aid training to residential staff
- Conduct medication management audits to promote safety and quality care
- Participate in a rotating after-hours on-call schedule (additional compensation provided)
What We're Looking For
- Active RN license or higher clinical licensure; BSN preferred
- 5–7 years of nursing experience preferred
- Experience in psychiatric, residential, or adolescent care strongly preferred
- Experience working with psychotropic medications strongly preferred
- Strong organizational, communication, and problem-solving skills
- Ability to remain calm and professional in challenging situations

- Ability to travel between residential group homes in Lexington, Nicholasville, and Georgetown
What We Offer

- A full-time position primarily based in Lexington, with travel to residential group homes
- A work cell phone and laptop provided
- Additional compensation for scheduled after-hours on-call responsibilities
