Nurse Coordinator

1st Call Home Healthcare is hiring a full time RN/LPN Homecare Nurse Coordinator to cover our West Michigan Territory. Must love to drive coverage area includes mostly Grand Rapids, with some Battle Creek and Kalamazoo areas. $$$ 5,000 Sign on Bonus $$$

The Nurse Coordinator assumes responsibility and accountability for the delivery of industry recognized home care nursing and health related services to the agency’s patients

Duties Include:

• Perform start of care visits and establish care plans. Monthly follow up visits.

• Perform Supervisory Visits to client and train and supervise caregivers

• Collaborate with interdisciplinary groups and coordinate care

• Handle emergency phone lines on off hours for medical emergencies

• Participate in community marketing and customer relation activities

• On occasion, assist Clinical Manager with Quality Assurance chart audits

What we offer:

Salary commensurate with experience beginning at $83,000 annually with $5,000 sign on bonus to be paid in instalments
