Nurse Coordinator

We are seeking a dedicated and dynamic Nurse Coordinator to join our healthcare team. Responsibilities

• Coordinate and manage patient care plans in a residential setting .
• Conduct thorough patient assessments, vital signs monitoring, and physiological evaluations to inform clinical decisions.
• Facilitate planning and transition of care processes, ensuring smooth handoffs to home health aides and CNA’s, or outpatient services.
• Collaborate with healthcare professionals to develop individualized treatment strategies that incorporate medical terminology and health regulation policies.
• Oversee clinical case management activities, including utilization management and diagnostic evaluation to optimize resource use and patient outcomes.
• Maintain accurate electronic health records (EHR/EHR management), medical records, and ensure compliance with HIPAA regulations for patient privacy.
• Engage in patient service activities such as intake assessments, managing patients as a nurse case manager, and providing education on medical procedures and health management and oversee CNA training.

Skills

• Extensive critical care experience with familiarity in ICU, PICU (Pediatric Intensive Care Unit), or Level I/II trauma center environments.
• Proven experience in emergency room settings, hospital medicine, or outpatient clinics with acute care expertise and or senior care.
• Strong knowledge of infectious disease care, geriatrics, behavioral health, and post-acute care practices.
• Proficiency in medical-surgical nursing, managed care operations, and hospital experience including discharge planning and medical records management.
• Skilled in utilizing electronic health records (EHR/EHR management), diagnostic tools, vital signs monitoring, and clinical case management software.
• Excellent client communication skills with the ability to collaborate effectively with healthcare professionals across disciplines.
• Deep understanding of physiology knowledge, health regulation policies, HIPAA compliance requirements, and medical terminology.
