Full Time | Days | Monday-Friday

Pay: $55 - $58/hour (B**ased on experience)

Lutheran Social Services of Central Ohio is currently seeking a Nurse Practicioner for LSS Health Center (formerly Health Center at Faith Mission) in Columbus, Ohio. What will I do as a Nurse Practicioner with Lutheran Social Services?

• Provides primary medical care evaluation, diagnosis, treatment, referral, and follow-up services.
• Creates treatment plans and works with team to make services and supports available.
• Orders and interprets diagnostic and therapeutic tests.
• Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
• Personally furnishes prescription medications from pharmacy.
• Provides acute and emergency care in the Health Center.
• Responds to emergency requests for consultation after regular business hours when available.
• Provides health screening, patient education, and preventative care.
• Documents all services in the Electronic Health Record in a timely manner.
• Monitors the effectiveness of interventions and participates in quality improvement, peer review, and evaluation activities.
• Collaborates with multidisciplinary team members and community partners and makes referrals to specialty care providers.
• Updates job knowledge by participating in educational opportunities.
• Performs other duties as assigned.

Requirements for a Nurse Practicioner with Lutheran Social Services:

• Graduate of an accredited school of nursing.
• Graduate of an accredited Nurse Practitioner Program.
• Current Advanced Practice Registered Nurse (APRN) Licensure.
• Current Nurse Practitioner certification in the State of Ohio.
• Current provider card in Basic Life Support (BLS)
• Experience with electronic health record.
• Strong computer skills, including Word, Excel, and PowerPoint.
