Nurse Practitioner 100% Virtual, CareBridge

For our client, we are seeking a Nurse Practitioner 100% Virtual Carebridge to join the team of a leader in the Hospitals & Medical Centers space. This role will provide business-aligned guidance focused on governance, compliance, and operational risk management. You will partner with leaders, compliance stakeholders, and cross-functional teams to translate complex issues into practical recommendations. The position offers the opportunity to strengthen responsible execution, improve decision-making, and support sustainable growth within a healthcare environment.

Location: Remote - US based candidates only, no visa sponsorship available

Compensation: $100,000 – $120,000 annually

Responsibilities

• Provides primary care via telephone and televisits to home and community-based patients
• Develops and implements clinical care plans for patients with chronic conditions
• Conducts patient history, physical exams, and diagnostics to inform treatment plans
• Identifies and addresses gaps in patient care
• Addresses patients' and families' psychosocial and physical needs with an interdisciplinary team
• Educates patients on medication, illness management, and dietary guidelines
• Maintains medical records in compliance with regulations and standards

Qualifications

• Master's degree in Nursing required
• Active and unrestricted NP license in Texas, Kansas, Iowa, or Arizona required
• Active RN Compact license required
• 2+ years of experience managing complex care cases necessary
• Experience with Electronic Medical Records (EMR) needed
