Nurse Practitioner, Advanced Practice Provider

Role: Nurse Practitioner, Advanced Practice Provider responsible for delivering equitable, value-based primary care to adult Medicare patients.

Responsibilities: Conduct office visits for routine and acute issues, administer wellness visits and health assessments, coordinate care with other providers, educate patients, participate in population health initiatives, and assist with phone triage, outreach, and home visits. Collaborate within a multidisciplinary outpatient team and work closely with specialists and hospitals to ensure comprehensive patient care.

Qualifications: Master of Science in Nursing, Doctor of Nursing Practice, or NP certification; active, non-probationary state NP license; active DEA license; US work authorization.

Preferred Skills: Experience in primary care (internal medicine, geriatrics, family medicine), knowledge of HCC, ICD-10 coding, and health risk assessments; strong teamwork orientation; bilingual proficiency is a plus.
