Nurse Practitioner

We’re Hiring: Nurse Practitioner – Full-Time or Part-Time

We are looking for a compassionate and motivated Nurse Practitioner to join our growing Primary Care team. This position is available full-time or part-time and includes the opportunity to provide care both in our office and in patients’ homes.

Our goal is to make high-quality primary healthcare accessible to everyone — including patients who may have difficulty traveling to a traditional medical office.

135 E. Minnesota Ave.
Orange City, FL 32763

In addition to office-based primary care, we provide in-home primary care services throughout Volusia. Our providers care for patients in private homes, Assisted Living Facilities (ALFs), and other residential settings.

Our mobile providers are supported by an Electronic Medical Record system and a full office staff that assists with scheduling, coordination of care, and patient needs. Our goal is to provide the same comprehensive, fully functional Primary Care experience in the patient’s home that they would receive in our office.

Primary Care services may include:

• Medication management and prescribing
• Chronic disease management
• Acute illness evaluation and treatment
• Wound care
• Preventive and routine primary care
• Coordination of patient care
• Office and/or home-based patient visits
