Job Summary

Responsibilities

• Conduct thorough patient assessments, including physical examinations and medical histories, with a focus on pediatrics, senior care, and acute care cases
• Develop and implement individualized treatment plans that incorporate pain management strategies and medication administration
• Collaborate with multidisciplinary teams to coordinate case management efforts and ensure seamless patient care transitions
• Manage patient documentation accurately using electronic health record systems such as Epic or Cerner
• Perform procedures such as phlebotomy and laboratory specimen collection with precision and adherence to safety protocols
• Provide education to patients and their families about health conditions, medication use, and preventive care measures
• Maintain current knowledge of medical terminology, assisted living protocols, dementia care practices, and home care procedures to deliver expert-level support

Requirements

• Valid Nurse Practitioner license with current certification to practice in your state
• Proven experience working with children, seniors, or in hospital settings with a strong emphasis on patient-centered care
• Experience in nursing home environments or assisted living facilities is highly desirable
• Background in acute care settings including emergency departments or hospital units
• Familiarity with medical terminology, medication administration protocols, and electronic health record platforms like Epic or Cerner
• Hands-on experience in pain management, case management, dementia care, laboratory procedures, and home health services
• Excellent communication skills for effective patient education and interdisciplinary collaboration
• Ability to handle multiple priorities efficiently while maintaining attention to detail
