We are seeking an Experienced (2+ Year Out Patient primary care) dynamic and compassionate Nurse Practitioner to join our healthcare team, dedicated to delivering exceptional patient care across diverse clinical settings. Responsibilities:

Nurse Practitioner (NP) – Outpatient Family Medicine

• Provide comprehensive primary care to patients across the lifespan in an outpatient family medicine setting.
• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Perform annual wellness visits, preventive screenings, physical examinations, and patient education.
• Order and interpret laboratory and diagnostic tests.
• Prescribe medications and develop evidence-based treatment plans.
• Manage chronic diseases, including hypertension, diabetes, hyperlipidemia, asthma, and COPD.
• Coordinate referrals and follow-up care with specialists.
• Document patient encounters, review lab results, manage medication refills, and respond to patient messages using eClinicalWorks (eCW) EMR.
• Collaborate with physicians and the interdisciplinary healthcare team to provide high-quality, patient-centered care.
• Ensure compliance with clinical guidelines, HIPAA, and quality standards.

Pay: $115,208.70 - $155,000.00 per year
