Wound Care Nurse Practitioner (NP)

About the Role

LaVida Heal PLLC is a growing Colorado wound care group providing evidence-based care across skilled nursing facilities, long-term care, and outpatient settings. We are looking for a dedicated NP or PA to join our team and help deliver high-quality wound care to patients throughout Colorado.

Responsibilities

• Evaluate and manage complex wounds, including pressure injuries, diabetic ulcers, surgical wounds, and chronic wounds.
• Provide wound care in SNF, LTC, and/or outpatient settings.
• Document patient visits and collaborate with facility staff, physicians, and clinical leadership.
• Use evidence-based protocols and advanced wound care modalities.
• Exercise independent clinical judgment while working with the support of a medical director and lead NPs.

Qualifications

• Active Colorado NP or PA license.
• Wound care certification (CWOCN, CWS, or WCC) preferred, or willingness to obtain certification.
• SNF, LTC, or wound care experience preferred.
• Valid driver's license and reliable transportation.
• Strong communication skills and the ability to work independently.
