Most patients finish a course of semaglutide or tirzepatide with no structured plan for what comes next and regain follows. We run a clinician-guided step-down and maintenance program that helps patients taper thoughtfully, protect lean mass, and hold onto the habits and results they worked for. Our platform combines virtual clinical oversight, nutrition and strength programming, and ongoing biometric tracking.

The Role

We're hiring PRN Nurse Practitioners to conduct assessments for patients entering our maintenance program. This is intake-focused work: you'll be the first clinician a patient meets, evaluating their GLP-1 history, current health status, and readiness to taper, then establishing the clinical plan our care team carries forward.

Visits are conducted via our telehealth platform on a schedule you set. There is no panel to manage, no call rotation, and no ongoing patient load — you pick up assessment blocks as your availability allows.

What You'll Do

• Conduct comprehensive virtual initial assessments, including weight history, GLP-1 dosing history, tolerance, and response
• Review medical history, current medications, comorbidities, and relevant labs; order baseline labs as indicated
• Screen for contraindications, disordered eating patterns, and conditions requiring escalation or referral
• Assess readiness for taper and develop an individualized step-down and maintenance plan
• Set body composition, nutrition, and activity targets in collaboration with our health coaching team
• Counsel patients on realistic expectations for the maintenance phase, including appetite changes and weight regain risk
• Document accurately and completely in our EMR within [24/48] hours of each visit

Qualifications

Required

• Active, unrestricted NP license in NY/NJ state
• Current national board certification (FNP-C/FNP-BC, AGNP, or ANP)
• Minimum [2] years of clinical experience as an NP
• Direct experience prescribing and managing GLP-1 receptor agonists
• Comfort with telehealth delivery and EMR documentation

Preferred

• Background in obesity medicine, endocrinology, bariatrics, or primary care weight management
• Obesity Medicine Association membership or ABOM certification
• Familiarity with body composition metrics and lean mass preservation strategies
