Full-time: Monday – Friday and alternating weekends

Nurse Practitioner Job Description

Maple Manor Rehabilitation Center is seeking a dependable and experienced Nurse Practitioner (NP) to provide high-quality medical care to residents in our skilled nursing facility.

The ideal candidate will be clinically knowledgeable, organized, comfortable working with geriatric and post-acute patients, and highly proficient with computers and electronic medical records (EMR).

Nurse Practitioner Responsibilities

• Provide medical evaluation and ongoing management of skilled nursing and long-term care residents.
• Perform routine, acute, and follow-up visits.
• Assess changes in resident condition and provide appropriate treatment and follow-up.
• Order and review laboratory testing, diagnostic studies, medications, and treatments as appropriate.
• Collaborate closely with physicians, nursing staff, therapy, pharmacy, and other members of the interdisciplinary team.
• Communicate with residents and families regarding medical conditions and plans of care.
• Participate in care coordination and hospital readmission prevention.
• Complete all required clinical documentation accurately and timely.
• Maintain compliance with facility policies and applicable state and federal regulations.

EMR & Documentation Requirements

Strong computer skills are required for this position. The Nurse Practitioner must be comfortable working independently within an electronic medical record system.

Candidate must be able to:

• Navigate and document efficiently in an EMR system.
• Complete progress notes, assessments, orders, and other required documentation electronically.
• Dictate clinical notes using voice-to-text/dictation technology when appropriate.
• Review resident charts, laboratory results, medications, and clinical documentation electronically.
• Complete documentation accurately and within required timeframes.

Job Requirements

• Valid Nurse Practitioner license in Michigan
• A clinical background in adult, family or geriatric specialties
• Minimum 2 years experience working in a nursing home or with seniors in other settings
• Must possess excellent verbal and written communication skills, excellent analytical skills, and excellent interpersonal relationship skills.
• Must possess clinical knowledge facilitating appropriate inquiry related to patient care.

Job Types: Part-time, Full-time

Pay: $35.00 - $58.00 per hour

License/Certification:

• Certified Nurse Practitioner (Preferred)
