Nurse Practitioner

Exciting opportunity to join a busy medical practice in the Irondequoit, NY area and become part of an interdisciplinary team that provides patient care utilizing the Patient Centered Medical Home model. This practice has a combination of specialties: Family Medicine and Internal Medicine/Pediatrics (Med/Peds).

Responsibilities include: evaluation and management of acute/chronic illnesses, ordering/interpreting/communicating diagnostic tests and results, complete physical exams, Wellness visits, hospital/ED follow-ups and Transitional Care Management, collaboration with network based physicians and medical team regarding patient care, facilitating appropriate referrals to specialists or community-based agencies, opportunities for office based procedures, patient education/counseling intended to maintain or restore physical/emotional health and function, and educational support for various learners consistent with the goals of the UR teaching institution. The APP will establish a team supported patient panel.

Excellent communication and collaboration skills are essential with the ability to exercise independent judgement and initiative.

An APP mentoring program is available for all new hires along with thoughtful onboarding.

Salary Range $98,000.00 - $130,083.20 annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Job Requirements Education : Fulfilled the educational requirements to be a licensed PA (c ompletion of an AMA approved Physician Assistant program, registered in New York State) or c ertification as a Nurse Practitioner registered in New York State

Experience : 2 -3 years of experience as a Physician’s Assistant or Nurse Practitioner in in family practice or other primary care setting preferred.

License/Certification : Current NYS Physician’s Assistant license or Family Nurse Practitioner license. BLS Certification required. DEA required prior to start date. Advanced Cardiac Life Support Certification preferred.
