Program Overview

Square Medical Group (SMG) provides comprehensive outpatient psychiatric and medication management services for individuals with mental health conditions, substance use disorders, and co-occurring diagnoses. Position Summary

Square Medical Group is seeking a compassionate, team-oriented Psychiatric Nurse Practitioner to join our outpatient practice, primarily based at our Weymouth, MA location. This position focuses on psychiatric evaluations, medication management, and treatment for individuals with mental health conditions, substance use disorders, and co-occurring diagnoses.

The Nurse Practitioner will work collaboratively with a multidisciplinary team and senior psychiatrists to provide high-quality, patient-centered care. Providers newer to the field will have opportunities to develop their clinical expertise through on-the-job training and clinical supervision, particularly in substance use disorders and dual diagnosis. Experienced providers will have the opportunity to apply and further expand their knowledge.

This is a hybrid position combining in-person care at the Weymouth outpatient location with telehealth services from a home office. Although the primary focus is outpatient care, the position may occasionally involve working within SMG's residential programs as organizational needs evolve.

Schedule Options

This is a full-time position with one of the following hybrid scheduling arrangements:

• Option 1: Four-Day Schedule (4/10) – Four 10-hour workdays per week, consisting of three in-person days and one remote day. The scheduled day off cannot be Monday or Friday.
• Option 2: Five-Day Schedule (5/8) – Five 8-hour workdays per week, Monday through Friday, consisting of three in-person days and two remote days.

The schedule will be established based on operational needs and mutual agreement.

Key Responsibilities

• Conduct comprehensive psychiatric evaluations and assessments for individuals with mental health conditions, substance use disorders, and co-occurring diagnoses.
• Provide ongoing psychiatric medication management, including medication initiation, monitoring, and adjustment as clinically appropriate.
• Provide medication-assisted treatment (MAT), including injectable medications, as appropriate to the provider's scope of practice and clinical privileges.
• Develop, implement, and monitor individualized treatment plans.
• Monitor treatment effectiveness, medication adherence, potential side effects, and changes in clinical presentation.
• Coordinate care and communicate effectively with referring providers, other members of the treatment team, and clients' family members, as appropriate and consistent with confidentiality requirements.
• Maintain timely and accurate clinical documentation in accordance with organizational policies and applicable regulations.
• Collaborate with psychiatrists and other clinical professionals to support continuity and quality of care.
• Participate in provider meetings, required training, clinical supervision, and other professional development activities.
• Serve as the on-call provider for a designated month.
• Adhere to all SMG policies, procedures, and applicable professional and regulatory standards.

Qualifications and Requirements

• Active Massachusetts licensure as a Psychiatric Mental Health Nurse Practitioner (PMHNP), Advanced Practice Registered Nurse (APRN), or Registered Nurse Clinical Specialist (RNCS), as applicable to the position and scope of practice.
• Current Drug Enforcement Administration (DEA) registration.
• Current Massachusetts Controlled Substances Registration (MCSR).
• Ability and willingness to provide medication-assisted treatment (MAT), including injectable medications.
• Strong clinical assessment, diagnostic, and medication management skills.
• Interest in and willingness to work with individuals experiencing substance use disorders and co-occurring mental health conditions.
• Ability to work collaboratively within a multidisciplinary clinical team.
• Willingness to work in outpatient and, if needed, residential settings.
• Strong communication, organization, and documentation skills.
