· Participate in interdisciplinary care planning and case conferences

· Contribute to a collaborative, collegial, interdisciplinary care environment

· Support the education and mentorship of learners

· Maintain accurate and up-to-date electronic medical records for all patient interactions

· Support organizational quality improvement activities and initiatives

· Unrestricted registration, in good standing as an RN (Extended Class) through the College of Nurses of Ontario (CNO). Master’s preparation preferred

· Active membership in NPAO and/or RNAO

· Professional liability insurance

· Minimum of three (3) years of NP experience in Primary Care setting

· Demonstrated critical thinking, problem-solving, organizational and interpersonal skills

· Excellent clinical, diagnostic, and decision-making skills

· Commitment to providing compassionate, evidence-based, patient-centered care.

· Ability to work both independently and collaboratively within an interdisciplinary team

· Sound judgement, ethical practice and professional conduct

Additional Requirements:

· Current BCLS

· Police Vulnerable Sector Check clearance (obtained at candidate’s expense)

· Proficient in the use of Electronic Medical Records (preferably Telus PS Suite)

· Organized, efficient, and self-directed

· Demonstrated understanding of and commitment to continuous quality improvement

· Valid Ontario driver’s license and vehicle insurance

· Commitment to ongoing professional development and training
