Job Description:

Forme Medical Center is currently seeking a Nurse Practitioner - INTERNAL MEDICINE - who is passionate, committed and enjoys working in a fast-paced environment. The ideal candidates are skilled in their ability to work independently, evaluating and treating a broad range of patients; prescribing medications and treatments, ordering, conducting, and interpreting diagnostic tests, such as x-rays, EKGs and lab work.

Qualifications

Requires degree or equivalent in education and experience, two plus years of experience

• Current New York State NP license
• Current BLS through AHA Certifications are required
• Current DEA, NPI certifications
• Current National Board Certification required
• Successful completion of applicable compliance and systems training requirements

Other Qualifications: Fluent Bilingual Preferred (English/Spanish)

Responsibilities:

The Nurse Practitioner - INTERNAL MEDICINE - will work with patients ranging from 18 to advanced age

• Order and analyze diagnostic tests
• Counsel patients about options for their medical conditions
• Provide non-life-threatening emergent care to patients of all ages, and management of acute and chronic illnesses.
• Conduct comprehensive health assessments, including physical examinations, medical history reviews, and ordering and interpreting diagnostic tests.
• Develop and implement treatment plans based on evidence-based guidelines and patient-specific needs.
• Manage overall patient care
• Educate patients on preventative care and disease management
• Accurate and clear updates and records changes in health status in EMR
• Make appropriate referrals to other health professionals and community agencies among others
• Must have a high degree of professionalism in all communications
• Excellent communication skills with ability to work individually and as part of a team
• Understanding that the patient is our highest priority

Job Type:

• Hourly Rate: $75 to $80/hour
• Experience: 2 years minimum
• Facility Schedule: Monday through Saturday
