What You'll Do

• Round on patients daily, building real continuity of care
• Diagnose and manage a wide range of acute and chronic conditions
• Work hand-in-hand with interdisciplinary care teams and facility leadership
• Chart smarter, not harder — with AI-integrated EHR documentation built for speed

What You Bring

• Active or eligible Oregon NP/PA license
• DEA and national certification (or ability to obtain)
• SNF experience is a plus, but not required — New grads are welcome
