Tuya PA – Independent Physician-Owned Primary Care Practice

Tuya PA is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing primary care practice. We are an established, independent, physician-owned practice with a focus on adult and geriatric medicine. We also have an office in Vero Beach.

We are looking for a provider who is compassionate, dependable, and comfortable managing the broad range of conditions encountered in outpatient primary care. We believe that good primary care should be accessible, personal, and proactive.

About the Position

The NP/PA will provide comprehensive outpatient primary care while working closely with our physicians, other advanced practice providers, and clinical staff.

Responsibilities include:

• Evaluate, diagnose, and manage acute and chronic medical conditions
• Provide routine primary and preventive care
• Manage common chronic conditions including hypertension, diabetes, COPD, CHF, hyperlipidemia, and other conditions frequently encountered in older adults
• Perform appropriate follow-up after hospitalizations and emergency department visits
• Review laboratory, imaging, and diagnostic results and develop appropriate treatment plans
• Perform medication reconciliation and ongoing medication management
• Identify and address preventive-care needs and gaps in care
• Coordinate care with specialists, hospitals, home health agencies, and other healthcare professionals
• Maintain accurate and timely clinical documentation
• Educate patients and families regarding medical conditions, medications, prevention, and treatment plans
• Work collaboratively with the clinical team to provide patients with timely access to care

Qualifications

• Current Florida Nurse Practitioner or Physician Assistant license.
• DEA registration
• Strong clinical judgment and communication skills
• Ability to work independently while collaborating with physicians and other members of the care team
• Primary care, internal medicine, family medicine, or geriatric experience is preferred, but we are willing to consider the right candidate with strong clinical fundamentals and an interest in developing within primary care.

Pay: $110,000.00 - $125,000.00 per year
