Nurse Practitioner/Physician Assistant

We have an opening for an APP in our exciting geriatric mobile practice focusing on wellness and disease management in the geriatric community. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork.

Our focus is primary care in assisted living communities. We are offering a full time position (Mon-Fri 8hr/day) to an APP to better serve our clients medical needs.

Being a mobile practice we go out to Assisted Living and Memory Care units to provide primary care services. We go to 1 community each day and are in all of our facilities weekly. For this position, we need coverage in both Kokomo and Marion, IN. We are looking for someone with a strong background in primary care or inpatient medicine that is willing to travel to both locations.

We have a full time LPN covering each community and a full time APP that is usually over 2-4 communities. The LPN works as a combination of an administrative assistant as well as a case manager.

Starting pay is around $130k but with our incentive based structure, our APPs average around $170-180k/year. We will cover you under our malpractice insurance, provide you with a collaborating physician and reimburse for DEA/licensure renewals. We do NOT offer CME reimbursement nor do we have a 401k.

Due to the unique nature of the position, we require a 2 hour shadow with one of our training providers in either the north Indy or Muncie area. We thank you in advance for your willingness to participate. This is a way for us to ensure alignment on both sides and decrease turnover.

This position offers a lot of autonomy. Due to the complexity and autonomy of the position, we do NOT hire new graduates.

It is a very exciting practice that we have built over the past 10 years. We currently have successful practices throughout Indiana, Ohio, Illinois, Kentucky, West Virginia and are opening practices in Tennessee this year. We are the preferred provider or medical director in each community we serve. Our reputation in the industry is vastly growing with many more communities ready to come on board this year.
