Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support using appropriate patient transfer methods such as Hoyer lifts.
• Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates, ensuring proper documentation within electronic health record (EHR) systems.
• Provide specialized care for patients with dementia or Alzheimer's disease, utilizing memory care techniques and ensuring a safe environment.
• Maintain a clean and safe environment by following infection control practices and safe lifting procedures during patient transfers.
• Communicate effectively with patients, families, and healthcare team members to ensure comprehensive patient service and understanding of care plans.

Qualifications

• Valid certification as a Certified Nursing Assistant (CNA) from an accredited program.
• Experience working with geriatric patients in long-term care facilities or nursing homes is highly preferred.
• Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS) certification.
• Prior experience in diverse healthcare settings such as assisted living facilities, rehab centers or memory care is advantageous.
• Familiarity with electronic health records (EHR/Epic/eClinicalWorks) systems for accurate medical documentation.
• Ability to perform heavy lifting safely and utilize manual handling techniques effectively.
• Strong caregiver communication skills coupled with a compassionate approach to patient care across pediatric to senior populations.
