Duties

• Conduct comprehensive patient assessments, including nutritional analysis and diagnostic evaluation, to identify individual dietary needs and health risks
• Develop and implement personalized care plans that incorporate medical nutrition therapy (MNT), weight management strategies, and chronic disease management protocols
• Provide nutrition counseling using motivational interviewing techniques to motivate clients toward healthier behaviors
• Collaborate closely with healthcare professionals such as physicians, dietitians, and nurses to coordinate patient care and optimize treatment outcomes
• Manage electronic health records (EHR/EHR systems) accurately for documentation of patient progress, care plans, and nutritional interventions
• Supervise dietary department activities related to clinical nutrition services and ensure compliance with hospital or long-term care standards
• Educate patients on specialized nutrition support methods including tube feeding, dietary modifications for geriatrics, and acute care nutritional needs

Skills

• Extensive knowledge of physiology, clinical nutrition, and medical nutrition therapy (MNT) applicable across diverse populations including geriatrics and individuals with specific dietary needs
• Proven experience in supervising staff within a healthcare or dietary setting, demonstrating leadership in a multidisciplinary environment
• Strong client communication skills with the ability to explain complex nutritional concepts clearly and empathetically
• Expertise in using EMR/EHR systems for documentation, tracking patient progress, and managing electronic health records efficiently
• Skilled in conducting patient assessments related to weight loss, chronic disease management, and acute care scenarios
• Experience working with individuals requiring specialized nutrition support such as tube feeding or medical nutrition therapy (MNT)
