Job Overview

True Harmony Home Health, LLC. is an ACHC accredited Home Health Agency that is seeking a dedicated and compassionate Nevada State Licensed Occupational Therapist to join its team. The ideal candidate will have at least 3 years of Home Health Care experience and a strong background in providing therapeutic services to patients across various settings, including inpatient and outpatient care. This role involves developing and implementing individualized care plans to enhance the functional abilities of patients, particularly those with various specific needs. The Occupational Therapist will work collaboratively with healthcare professionals to ensure optimal patient outcomes.

Responsibilities

• Conduct comprehensive assessments of patients' physical, emotional, and social needs.
• Develop and implement personalized care plans tailored to each patient's unique requirements.
• Provide therapeutic interventions aimed at improving daily living skills and overall quality of life.
• Collaborate with interdisciplinary teams to coordinate patient care and share progress updates.
• Educate patients and their families about treatment plans, goals, and strategies for continued improvement.
• Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
• Maintain accurate documentation of patient assessments, treatments, and outcomes in compliance with regulatory standards.
• Stay informed about advancements in occupational therapy practices, particularly in areas such as ICU care, early intervention, and outpatient services.

Requirements

• A degree in Occupational Therapy from an accredited program.
• Valid Nevada State license.
• At least 3 years of experience in Home Health Care.
• Strong knowledge of anatomy and physiology relevant to occupational therapy practices.
• Experience working with diverse populations, especially older adults.
• Familiarity with developing care plans that address specific patient needs in both inpatient and outpatient settings.
• Excellent communication skills for effective interaction with patients, families, and healthcare teams.
• Ability to work independently as well as collaboratively within a team environment.
• A commitment to providing high-quality care while demonstrating empathy and understanding towards patients.
