Occupational Therapist

Optimize Home Health is looking for an Occupational Therapist to join our growing home health team.

We are looking for an OT who enjoys working independently and wants flexibility with their schedule and caseload. You will provide occupational therapy services to patients in their homes and have flexibility in choosing your availability and the areas you are comfortable covering.

Previous home health experience is helpful, but it is not required. If you are new to home health, we are happy to provide training and help you become comfortable with home health documentation, scheduling, and patient care.

Responsibilities:

• Complete occupational therapy evaluations and re-evaluations
• Develop individualized plans of care and treatment goals
• Provide skilled occupational therapy services in the patient's home
• Educate patients, families, and caregivers
• Complete required documentation in a timely manner
• Communicate with physicians and other members of the care team
• Complete discharge assessments when appropriate
• Coordinate care with PT, nursing, and other disciplines involved with the patient

Requirements:

• Current California Occupational Therapist license
• Valid driver's license
• Reliable transportation
• Ability to travel locally to patients' homes
• Good communication and documentation skills

New graduates and Occupational Therapists without previous home health experience are welcome to apply.

Pay: $120,000.00 - $200,000.00 per year
