Responsibilities

• Perform comprehensive occupational therapy evaluations and provide skilled treatment in patients' homes.
• Develop and implement individualized plans of care to improve independence, safety, and participation in daily activities.
• Provide therapeutic interventions addressing activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, coordination, strength, and functional mobility.
• Assess patients' home environments and recommend adaptive equipment, assistive devices, and environmental modifications to improve safety and independence.
• Educate patients and caregivers on home exercise programs, energy conservation, fall prevention, and adaptive techniques.
• Monitor patient progress and modify treatment plans as clinically appropriate.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with applicable regulations and agency policies.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care.
• Complete evaluations, reassessments, progress notes, and discharge documentation in a timely and compliant manner.

Requirements

• Current California Occupational Therapist license.
• Home health experience preferred; training and support available.
• Reliable transportation.
• Strong communication, organizational, and time management skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to providing compassionate, patient-centered care.

Optimize Home Health is seeking Occupational Therapists to provide coverage throughout Greater Sacramento and surrounding Northern California communities.

• Greater Sacramento: Sacramento, West Sacramento, South Sacramento, Natomas, Arden-Arcade, Carmichael, Fair Oaks, Citrus Heights, North Highlands, Rio Linda, Antelope, Orangevale, Rosemont, Florin, Rancho Cordova, Gold River, Elk Grove, and Folsom.
• Placer County: Roseville, Rocklin, Lincoln, Granite Bay, Loomis, Auburn, Newcastle, Penryn, Applegate, Meadow Vista, and Colfax.
• Nevada County: Grass Valley, Nevada City, Penn Valley, and surrounding communities.
• Yolo County: Davis, Woodland, Winters, and surrounding communities.
• Colusa County: Colusa, Williams, Arbuckle, Maxwell, and surrounding communities.
• Yuba & Sutter Counties: Yuba City, Marysville, Linda, Olivehurst, Live Oak, Wheatland, and surrounding communities.

You Choose Your Coverage Area!

We understand that every therapist has different scheduling needs and geographic preferences. You can select the areas you are comfortable covering and accept patient referrals based on your availability.

You are not required to travel outside your preferred coverage areas, and you have the flexibility to accept as many patients as your schedule allows.
