Occupational Therapist - Full Time

Position Overview An Occupational Therapist (OT) is responsible for assessing, teaching, and delivering therapy interventions within the home health setting, in collaboration with an interdisciplinary team. Key duties include performing initial and ongoing assessments of mobility, strength, and function; developing and updating individualized plans of care; documenting patient progress; and ensuring effective discharge planning with patient and caregivers. The OT provides therapy aligned with physician orders, maintains accurate documentation to avoid denials, and communicates effectively with team members and families. Supervision of other personnel such as OTA and HHA may be required, along with completing OASIS assessments where permitted by regulation.

Requirements

• Degree in Occupational Therapy from an accredited program
• Current, unrestricted state license as an Occupational Therapist
• Valid CPR certification
